Calgary Rural Primary Care Network (CRPCN)
Established in 2006, the Calgary Rural PCN is a group of family doctors, health care members, and Alberta Health Services (AHS) who work together to provide primary care throughout 15 core communities southeast and southwest of Calgary. For the Strathmore region our PCN team consists of health specialists like registered and licensed practical nurses, dietitian, family counsellor, client resource specialist, and an exercise specialist - all working together to ensure greater continuity of care for each patient. Between the one-on-one services they offer, and the free classes offered on nutrition, exercise, health, and mental wellness, we work with family doctors to ensure your best health outcomes.
The CRPCN helps people access the health services they need when they need them, by building ongoing relationships between patients, their family doctor, and the health care team. Plus, CRPCN knows their way around the health care system, and can help navigate your best path.
